What to Bring to Your Appointment
For your appointment, please bring the following documents:

- Referral: Ensure you bring the referral letter from your GP. If your GP referred directly to the practice, please contact us to confirm that we have received the referral.
- Patient registration form: Fill out your patient registration form in advance and bring a printed copy to your appointment. This form outlines your Medicare and private health insurance details.
- Medical information: Bring any relevant test results, ultrasounds, or x-rays. This information helps Dr Hankins better understand your medical history and provide appropriate care.
Obtain a GP Referral
Before your appointment, obtain a GP referral to Dr Kelly Hankins. This referral is necessary for Medicare rebates and typically lasts for 12 months. If you don’t have a referral letter, please contact your GP to arrange one.
Make Your Appointment
Once you have your GP referral, please contact the clinic at 02 4321 0500 to schedule your appointment. The friendly staff will assist you in finding a suitable time for your visit.
Cancellation Policy
We kindly request that you provide at least 24 hours’ notice if you need to cancel your appointment. This allows us to offer the slot to another patient requiring care.
